How do I create an induction form?

Building a custom induction form that captures the safety information and acknowledgements a worker or visitor must complete before accessing a site.

An induction makes sure every worker and visitor has the safety information they need before they start on site. You can build it from questions, documents and announcements such as videos or slideshows, and ask people to acknowledge what they've read.

This article shows you how to create an induction, choose when and where it's completed, and add your content.

Before you start

  • You need Company Host permissions. Only Company Hosts can create or change inductions. If you don't have this, ask your Breadcrumb administrator.
  • You can only create inductions on web. It isn't available in the mobile app.
  • The questions you want to use need to exist already. If you want to limit the induction to certain visitor types, set those up first too.

Create the induction

  1. Select Editor in the left-hand menu, then select Inductions.
  2. Select the blue New Induction button.

Set up the induction

Fill in the settings on the left-hand side of the page.

Basic details

  • Induction Title: the name workers will see.
  • Active: turn this on to make the induction available.
  • Expiry: how long a completed induction stays valid before it needs to be done again. Choose No Expiry, End of the Day, End of the Week, End of the Month, End of the Year or Custom to set your own period.

When it's completed

Use Perform Induction to choose when workers complete it:

  • Anytime: workers can complete it whenever they like.
  • Check-in: workers complete it when they check in to site.
  • Pre Check-in: you send workers a code so they can complete it before they arrive.
  • Offsite: you send workers a link or QR code so they can complete it before they arrive.

Turn on Is Mandatory? if workers must complete the induction before they can check in.

Where it applies

  • Company wide: applies to every site. Use this for training everyone needs, like your general safety induction.
  • Site Specific: covers what's particular to one site. You'll need to add it to each site yourself once it's created.

To show the induction only to certain people, turn on the visitor type option and choose the visitor types it applies to.

Approval and documents

  • Host Approval: turn this on if Company and Site Hosts should approve or reject each completed induction.
  • Allow users to attach additional documents: turn this on if workers should be able to upload documents, like a licence or competency, as part of the induction.

Add your content

Build the induction using the panel on the right-hand side.

  • + Add Questions: add questions from your question list.
  • + Add Documents: add documents workers need to read, like site rules or a SWMS.
  • + Add Announcements: add videos, slideshows or other announcements.

For each announcement, choose how workers need to interact with it:

  • Disable: viewing is optional.
  • Mandatory View: workers must open it.
  • Mandatory View and Sign Off: workers must open it and sign to acknowledge it.
  • Set Play Through To Mandatory: for videos, workers can't skip ahead and must watch to the end.

To change the order workers see things in, drag and drop questions, documents and announcements.

Preview and save

  1. Select Show Preview to see the induction the way workers will.
  2. Select Add to save it.
