How do I create a plant registration form?

Building the form used to register a piece of plant or equipment before it's brought onto site.

Before plant or equipment comes onto site, you'll want a record of what it is and that it's safe to use. A plant registration form captures those details, like the make, model and serial number, so every item on site is registered the same way.

This article shows you how to create a plant registration form and add the questions you need.

Before you start

  • You need Company Host permissions. If you don't have them, ask your Breadcrumb administrator.
  • You can only create plant registration forms on web. It isn't available in the mobile app.
  • The questions you want to use need to exist already.
  • If you also want operators to fill in their own form, create that first so you can link it. See Create an asset operator form.

Create the form

  1. Select Editor in the left-hand menu, then select Forms.
  2. Select New Form.

Set up the form

  • Name: the form's name, for example Plant Registration.
  • Button Title: the text workers see on the button in the app.
  • Type: select Asset Registration.
  • Scope: choose Company Specific to add the form to all your sites automatically, or Site Specific to choose which sites it appears on.
  • Ask Mode: choose when workers can fill it in: While Checked-in, After Check-out or Anytime. We recommend While Checked-in.
  • Is Active?: set to Yes to make the form available.
  • Operator Registration Form (optional): link a separate form for the plant's operator to complete.
  • Form Group (optional): group this form with related forms.

Add your questions

Select + Add Questions and choose the questions you want to include.

To capture details about the plant itself, use Asset Field questions. Each one links to a Global Asset Field, such as Make, Model or Serial Number, so the answers are saved against the plant's record.

Add announcements

Select + Add Announcements to show workers information as part of the form, like a safety video. Then pick how they need to interact with it:

  • Disabled: viewing is optional.
  • Mandatory View: workers must open it.
  • Mandatory View and Sign Off: workers must open it and sign to acknowledge it.
  • Set Play Through To Mandatory: for videos, workers must watch to the end.

Preview and save

  1. Preview the form to check it looks right.
  2. Select Add to save it.
