How do I create a permit form?
Building the application form workers submit to request a permit, such as hot works or confined space entry.
Some work needs a permit before it can start, like hot works or confined space entry. A permit form is the application workers fill in to request one. Once they submit it, your site management team reviews and approves it before the work goes ahead.
This article shows you how to create a permit form, set how the permit moves from approval to close, and add your questions.
Before you start
- You need Company Host permissions. If you don't have them, ask your Breadcrumb administrator.
- You can only create permit forms on web. It isn't available in the mobile app.
- The questions you want to use need to exist already.
- If you want separate forms for opening, putting on hold or closing the permit, create those first so you can link them.
Create the form
- Select Editor in the left-hand menu, then select Forms.
- Select New Form.
Set up the permit
Basic details
- Name: the permit's name, for example Hot Works Permit.
- Button Title: the text workers see on the button in the mobile app.
- Type: select Permit.
- Is Active: turn this on to make the permit available on site.
Who can apply
Use Apply For to choose who the permit is for: Anyone, an Individual or a Supplier.
What happens after approval
Choose the permit's status once it's approved:
- On-Hold (default): the permit stays on hold until someone opens it manually.
- Open: the permit opens automatically once it's approved.
How the permit is closed
Choose when a permit can be closed:
- Must be On Hold to close: the permit has to be put on hold before it can be closed.
- Can close while Open or On Hold: the permit can be closed from either status.
Linked forms
You can link a form to each stage of the permit. Use the Permit Open Form, Permit On-Hold Form and Permit Close Form dropdowns to pick the form that needs to be filled in at that stage.
Other options
- Can Set Schedule: enable this to let workers set a schedule for the permit.
- Can Request Exclusive Access: enable this to let workers request exclusive access as part of the permit.
- Form Group (optional): group this permit with related forms.
- Visitor Types (optional): only show the permit to certain visitor types.
- Attachments: let workers attach documents, like a SWMS, to their application.
Add your content
Questions
Select + Add Questions and choose the questions workers need to answer to apply.
Announcements
Select + Add Announcements to show workers information as part of the application, then pick how they need to interact with it:
- Disable: viewing is optional.
- Mandatory View: workers must open it.
- Mandatory View and Sign Off: workers must open it and sign to acknowledge it.
- Set Play Through To Mandatory: for videos, workers must watch to the end.
Preview and save
- Select Show Preview to check the form looks right.
- Select Submit to save it.
