How do I create a generic form?

Building a custom form from scratch for any purpose not covered by Breadcrumb's other form types.

A generic form is a form you build yourself for anything Breadcrumb's other form types don't cover. Think daily prestarts, hazard reports, toolbox talks or quality checklists. Workers fill it in from the Breadcrumb app on site.

This article shows you how to create a generic form, set who sees it and when, and add your questions.

Before you start

  • You need Company Host permissions. If you don't have them, ask your Breadcrumb administrator.
  • You can only create forms on web. It isn't available in the mobile app.
  • If you want to limit the form to certain visitor types, set those up first.

Create the form

  1. Select Editor in the left-hand menu, then select Forms.
  2. Select New Form.

Set up the form

Fill in the form's settings:

  • Name: the form's title.
  • Button Title: the text workers see on the button in the app. It can be the same as the name.
  • Type: select Generic.
  • Scope: choose Company Specific to add the form to all your sites automatically, or Site Specific to choose which sites it appears on.
  • Ask Mode: choose when workers can fill it in: While checked-in, After check-out or Anytime. We recommend While checked-in.
  • Active: set to Yes to make the form available. Set it to No if you're not ready to use it yet.
  • Form Group (optional): group this form with related forms.
  • Visitor Types (optional): only show the form to certain visitor types.

Add your content

Questions

Select + Add Questions and choose the questions you want to include.

Announcements

Announcements let you show workers information, like a document or video, as part of the form. Select + Add Announcements, choose an announcement, then pick how workers need to interact with it:

  • Disable: the announcement isn't shown.
  • Mandatory View: workers must open it.
  • Mandatory View and Sign Off: workers must open it and sign to confirm they've read it.
  • Set Play Through To Mandatory: for videos, workers must watch to the end.

Preview and save

  1. Select Preview to check the form looks right.
  2. Select Submit to save it.
