How do I create a document review (e.g. SWMS, or SDS) checklist?

Building the checklist used to review a submitted compliance document, such as a SWMS or SDS, before it's approved.

When a subcontractor or supplier submits a compliance document, like a SWMS or SDS, someone on your team needs to check it meets your standards before it's approved. A document review checklist gives your reviewers the same set of questions every time, so nothing gets missed.

This article shows you how to build a review checklist and link it to the document types it should be used for.

Before you start

  • You need Company Host or Site Host permissions. If you don't have them, ask your Breadcrumb administrator.
  • You can only create review checklists on web. It isn't available in the mobile app.
  • Add your review questions to the question list first. See Create a question.
  • If you want reviewers to see a PDF, video or other document as part of the checklist, add it as an announcement first. See Create an announcement.

Create the checklist

  1. Select Editor in the left-hand menu, then select Forms.
  2. Select New Form.
  3. Set Type to Supplier Document Review.
  4. Enter a name for the checklist, for example SWMS Review.

Choose which documents it's for

Use Supplier Document Kind(s) to choose the document types this checklist should be used to review, for example SWMS or SDS. You can select more than one if the same checklist works for several document types.

Add your content

  • Add Question: add the questions reviewers need to answer, for example "Does the SWMS cover all high-risk work on this site?"
  • Add Documents or Add Announcements: add any reference material reviewers should see while they check the document.

Save the checklist

Select the blue Add button to save.
