How do I add a form to my site?
Attaching an existing form to a site so it is required as part of check-in or onboarding.
Forms in Breadcrumb are set up as either Company Wide or Site Specific. Company Wide forms are added to every site automatically. Site Specific forms aren't, so you choose which sites they appear on.
This article shows you how to add a Site Specific form to a site, and how to remove it when it's no longer needed.
Which forms this applies to
You can add these form types to a site:
- Generic forms
- Permits
- Asset registration forms
- Site briefings

Before you start
- You need Company Host or Site Host permissions, with permission to manage sites. If you don't have these, ask your Breadcrumb administrator.
- The form must already exist and be set to Site Specific in the Editor. If it isn't, create it first. See Create a generic form or Create a permit form.
- You can only do this on web. It isn't available in the mobile app.
Add a form to a site
- Select Sites in the left-hand menu.
- Find your site and select Manage Site.
- Scroll down to the section for the form type you want to add, for example Permits.
- Select the blue + Add button for that form type.
- In the pop-up, find the form and select + next to it.
- Select Submit Changes to save.
Remove a form from a site
- Select Sites in the left-hand menu.
- Find your site and select Manage Site.
- Scroll to the form type section and find the form.
- Select the red Remove button next to it.
- Select Submit Changes to save.
